Creating an open reporting culture in the NHS

Sir Robert Francis QC is heading up an independent review into creating the open and honest reporting culture in the NHS. This was announced by the Secretary of State on 24 June 2014. It is called the Freedom to Speak Up Review; further details can be found here
The review is looking for personal experiences and views, experiences of speaking up and raising concerns, what was good or bad about the experience and ideas about improvements that could be made. The review needs contributions by 10 September 2014. Details on how to do this can be found on the Freedom to Speak Up website
