With more than 300 million patients consulting with primary care teams annually it’s unfortunately inevitable that a proportion will suffer some form of unintentional harm, mostly of low to moderate severity. Research has suggested that around 1-2% of consultations in primary care are associated with an adverse event. The cost of harm – to patients, to those working in healthcare, and to productivity – is significant.

Creating a positive safety culture can be achieved by avoiding complacency and encouraging vigilance towards patient safety. Such a culture means that people feel comfortable discussing errors, and leaders and frontline staff take shared responsibility for delivering safer care. This in-house workshop explores important issues and skills in relation to learning from patient safety incidents and will provide guidance on how to hold a significant event meeting.
